accountability. To this end AchieveAbility has developed the AchieveAbility Family
Self- Sufficiency Continuum.
AchieveAbility Family
Self-Sufficiency Continuum a tool for client evaluation and program
evaluation AchieveAbility evaluates the success of
the Family Self-Sufficiency Program by the movement of families along the Self-Sufficiency Continuum.
It is an objective, measurable tool that
both informs and reflects a family's progress from dependency to self-sufficiency.
The continuum contains objective, measurable criteria for education,
parenting, finances and personal development. Movement upward along the
continuum, from level one through level five, represents success and accomplishments
that will bring the family from depending on temporary assistance to
independence. Social workers also use the continuum to
work with the families -- the family reviews the continuum before the family even enters
the AchieveAbility program. Clear objectives are included
in the continuum for the family to work toward while participating in the AchieveAbility
program. These objectives, when attained, move the family closer and closer to
self-sufficiency. Quarterly reviews between a social
worker and a family use the continuum. During the quarterly review, the family
and the family's social worker reflect on the accomplishments and challenges
of the past three months, and update the family's goal plan accordingly. The
family and the social worker also rate the family's progress on the continuum.
This interactive process, which reflects and guides the family's progress, is
an integral part of the ongoing relationship between the family and the
family's social worker so there is a natural, unobtrusive program evaluation
tool as well. |
